13:00 - he tps mid and dies when there's 5 enemies but not enough our team members
14:00 - he dives beyond t1 tower and dies to their team
20:00 - he tps bot and dies again instead of defending behind tower
26:30~27:30 - he afks at base

~11:00+ Wyvern TP's mid against 3 and dies.
~12:40 Wyvern goes mid to help teamate, both dying 2 on 3 by Void's ult.
~22:00 Wyvern TP's bot into rhasta's ult and dies immediatly.
~26:30 to 27:30 Wyverns goes back to base after killing creeps @ T2 tower area, then afks for a whole minute and starts helping again.

What would you say is the optimal course of action for us to take here?
Did he die on purpose?

He didn't die on purpose the way I see it. He's just bad.
And that afk, he could of been waiting for his item, or went to bathroom. It was just a minute, no big deal.
I'd let that one go.

I agree with @Crows . None of his deaths was intentional. He was just a new player.
When he died by rhasta, he actually teleported to defend bot tower. Only after he teleported there, rhasta used his ulti on him.

Also, most of his deaths were by void. Not easy to counter that, especially for a new player.
The afk was not exactly afk. He was waiting to gather 1k gold, to buy his mask.


On the other hand, Kunkka spawns on 1300, and stays afk for a minute.
Then again on 1740, until 2250.
And again from 2500 to 2800.
Total of 9 minutes, in a 33 minutes game.

Kunkka is banned for 4 days.
