Ok, so this is a tricky one. My personal findings and opinion:

1) First votekick was initiated after lvl3 (during a leak) by green player at 08:59 replay time.

Teal player zynk@uswest.battle.net did command !afk, so he was aware of orange being afk. He also teased he will agree to VK when other team kick red player (new to the game and leaking badly, which isn't any offence). Noone agrees to VK.

2) Green player Serotonia5 tries to initiate second votekick, players agb544, zynk, Erwin type !yes, but since there is build-in delay on -votekick command it wasn't initiated.

I strongly believe this was joke VK by Zynk, he saw how much advantage he is gaining by not kicking, did the VK to not be accused of breaking the rules, he told his team to not say !yes:

Conclusions:
I agree with a ban for players: zynk@uswest.battle.net and player smoky[dib]@europe.battle.net for refusing to kick afker.
I would issue a warning for player agb544@uswest.battle.net for refusing to kick afker. Warning instead of ban because I can see him being new to the game and ENT, and also being very much confused by blue not talking at all, and teal misleading informations and teases.
