Time of Violation (in-game or replay): I will continue to watch this and add more stamps that I can find, but for the time being this should be more than obvious.
18:30 replay time. Warlock is sitting deep in our territory in between 2 towers and has not moved. The last vision of him is at 17:40 when he dies. At 18:30 tiny leaves his base and makes his way to river and crosses to scourge secret shop. At ~19:10 warlock revives and stops behind mid tower for a moment. 19:25 tiny proceeds to make his way from shop to mid lane and then blinks right on top of warlock and immediately tosses and stuns. No way he knew he was there.
Any further thoughts: On top of this, he tossed his allies into us several times when they were defending mid!

The above time stamp indeed is highly suspicious. Tiny blinks blindly right on top of warlock, and in approximately 1/2 second, avalanches and tosses. Too fast for reflexes if a blind lucky blink.

I found no other suspicious moves in this game however, so I reviewed this one:

849: gains vision of sk emerging from fog and turns to him in approximately 1/3 of a second.
853: disengeages circle of creeps to dodge wr arrow coming from fog.
1030: dodges wr arrow from fog.
1317: blinks to wr the instant she becomes visible. No reaction time.
1849: blindly blinks right at wr.
2913: clearly aware of wr's presence in fog, moving directly to her, using euls the instant he gains vision, no reaction time.
