Rule player violated: Intentionally throwing the game, after complaining about unplayable lag for the entire game. Opposing team would not !yes the votekick. Caused the DK to leave out of frustration.
Time of incidents (Note in-game timer or replay timer):
Game-timer - 2, begins notification of lag.
Game-timer - 4:45, says it's "not even fun" playing with the lag.
Game-timer - 8:30, announces ping of 300 ms
Game-timer - 18, blames deaths and inability to play on lag. Says "I can't do anything".
Game-timer - 26:50, again says "Can't do anything" blaming it on lag.
Game-timer - 32:55, due to lag admits that he cannot even stop running forward to his death and can't even use berserker's call.
Game-timer - 35:45, announces ping of 500 ms
Game-timer - 38, almost inadvertently throws the game for Sentinel
Game-timer - 45:47, calls us "noob" and continues to blame his gameplay on lag.
Game-timer - 46:38, !votekicked axe, but to no avail.
Game-timer - 49, does not join teamplay, then blames on lag again.
Game-timer - 50:25, second votekick, doesn't pass.
Game-timer - 52:05, Axe getting killed by meepo, has a blink dagger, and we tell him to blink, but instead he keeps running and dies. Then he goes on to farm neutrals after respawn and blames me for not ulting (potm) specifically to save him. Following that, DK rage-quits, and we lose the game.
Any further thoughts:
Started the game with decent/good team spirit with no flaming. We all tolerated Axe's gameplay and lag throughout the entire game until towards the end when he was clearly throwing the game and saying he was playing through unplayable lag. He can use his moves on creep without a problem, but struggles to use them on heroes when we are all alive. Throws the game because I did not ult to specifically save him and goes on to neutral creep when we try to push/defend; he admits all of it, check in-game text. The flaming I admit we did towards the end due to frustration, but it's not to say he didn't flame back at us. We already finished mid rax and were trying to throne the Scourge, when he decided to neutral creep (this is when we have lost two rax and bottom tower already. he would not even go defend bottom while we pushed). Our SA was not a good player, but at least he tried to help us instead of making BS excuses, and he did not flame or troll either. Throughout the entire game, we have been giving Axe constructive criticism and help only to have him shut us down on the win in an over 50 min. long game.

Also I forgot to mention. I have had experience getting severe lag up to 500 ms when i used !checkme, and my lag actually interrupts the gameplay for everyone as they see my countdown to drop; they get annoyed and would try to votekick. When axe "lagged" there was no "drop countdown" and we all played normally. He just fed and played stupidly, blaming it on lag, then he didn't even try to help. How hard can it be for an Axe to auto-attack creeps in order to defend a lane? Seriously?

Edit: The notes and times are accurate and this player's actions severely affected the game. His decision to stay in the game despite such "bad lag" definitely ruined the experience for the other players. He should have left or tried to fix his connection long before stay in a game for so long. The fact that he was 1-8 also doesn't help very much. The part that really gets me is that he made good items, so he obviously wasn't bad, but he did let his so called lag ruin the game.
