Rule player violated: game-ruining through feeding, assumed to be deliberate because we tried to help him in every way possible in how to play his hero and what items/skills to get, yet halfway through the game he types "?" and "help me". wouldn't swap meepo at the beginning of the game either, and wouldn't !ff when the whole team did.
Time of incidents (Note in-game timer or replay timer): Don't remember the specific times he fed, but his score was 1-9-1, and he was the lowest level despite his attempt to "farm". Continued to 1v4 and feed after the team typed !ff at the end of the game.
Any further thoughts: Is it possible at all to ban the players on sentinel? Their WW dc'ed and it was a 4v5; despite our meepo feeding and ignorance of team [positive] help, only one player types !yes after sentinel takes double rax. They refused to even the game to a 4v4 and continued to feed off of meepo. Reason: refusal to kick game-ruining player.

sone-1 is just a beginner. its right that your team told him right after start which items he should buy but thats it. The rest of the time you were just flaming him and about him.
beeing a beginner is no reason to ban. your votekick try was also invalid and you are lucky that he didnt pass.
Please keep in mind that only purposefull feeding is bannable. But there is one except: if you have a beginner/ unexperienced player in your team who feeds and you really try to help him and tell him what to do to evade further deaths and he ignores the tips, then he can be banned.
