Being bad at the game is not a banable offense (Atleast to my knowledge). You probably encountered me on a day where I was drunk or on drugs.

Looking over the replay, it does not appear that he missed any team sends, and spent his gold every round (minus level 12, however, he did ask for a pause to go to the washroom, but was met with "leave if you don't want to be IP banned").

Someone noting they are intentionally running games does not necessarily mean they are intentionally throwing. Player has a below 50% win rate, but seeing the amount of games and current ELO I'd believe he was messing with you at the time. Him noting that he's attempting to get the lowest ELO possible while currently sitting at 989 ELO (1000 is default) is not actually evidence enough to suggest he's throwing on purpose.
