To be honest, I am not sure if he has done this obviously enough to warrant being banned, but we all know what he's doing.

Every game, he picks some random hero, takes mid lane (usually it's already been called so he's dual mid), then build dagon as his first and only item, sits back for the entire game and steal kills.

Some recent obvious games:
https://entgaming.net/findstats.php?id=10440442 : 41 minute game, he ends 2/1/2. He never participated in a teamfight. Builds dagon5 first on jugger.
https://entgaming.net/findstats.php?id=10436004 : His only item is dagon5 and plain boots.
https://entgaming.net/findstats.php?id=10435925 : He picks riki and sits invis. Never even hits the enemies. Just uses dagon to finish them. So he ends with 10 kills 1 assist, but no doubt he made his team lose.

He does this literally every single game. He ruins every game he plays, no doubt because he wants to get a very low ELO account (around 500 now).

there is also about at least 20 other games that he does the same thing. He rushes dagon 5 in all of those other games and he does not help the team. He waits to ks. I played against him and i never even knew he was part of the game. Definitely should be warned and if he does not heed that warning, banning for a week or two may help so that he stops ruining games. I get it that someone isnt good at dota would find that getting dagon helps him get kills here and there but it also makes him lose 95% of them validating that he may be tanking elo.

Yeah. He steals mid, autoattacks creeps and lets the enemies kill him. Builds dagon 5. Kill steals, while sitting back from any real fights. Never participated in one fight the entire game.

I did flame the enemies a fair bit since they wouldn't kick him when he was clearly throwing. And then they lost to him anyways, which is a bit sad.
I out-carried a Void and Riki using leshrac. So I stand by what I said:

Before this, he had just wandered out into two enemies to die, and I saved him and said I wouldn't again.

just watched replay too, I think he's saying Lesh forcing him to die is a form of throwing, based off of what you're typing but it's difficult to really comprehend.

In all seriousness, this game you and UR.DEAD (I-AM-AMAZLNG) did jump on him quite a bit without reason, then MODZ the sniper also joined in trying to verbally attack him. I didn't watch beyond the point where the slark got upset and ignored everyone, so I wouldn't know if he broke any rules.

A bit of advice though, when I see players like this esp mexican_wisp or heyjude, just go next, let them fail/throw or whatever, it's not our responsibility to play with certain players.

1 year! Just ban every "nightmare" account for extra safety.

I watched the juggernaut, invoker and sa games.
In the juggernaut game he meets criteria for game ruin: no communication with allies. Sits on outside of team fights, spinning away at any hint of trouble, and basically accomplishing nothing.
In the invoker game, he meets criteria for failure to defend when he goes afk in jungle at the end of the game while the fountain is destroyed.
In the sa game he also meets criteria for game ruin/failure to cooperate with allies. He seldom uses smoke, he wanders the map invisible, not creeping, not gaining exp or gold. He would hover around the edges of team fights, darting in to dagon then run.

nightmares banned 3 days for game ruin/failure to cooperate with allies/failure to defend.
