1st Game Review:
Saw no intentional feeding. Only saw two occasions early on game where he should have retreated since his health was in red/yellow but chose to stay to farm gold which resulted in his death. Second occasion was probably due to bickering back and forth between the two players and then slark got him from behind.
Other than that, he was in most of the fights aiding his teammates and several other deaths occurred in 1v1 battles which he probably thought he could win and does come close too.
He also does attempt to last hit for gold, escape when he can, and took down several towers.
He's just a very sarcastic person and both players obviously didn''t get along.
For instance, you astral imprisoned him for no reason at mid, then he used eclipse to farm creeps. 10sec or so later slark attacks from within tree and kills him.
Ban request for game ruin/feeding should be denied!
As for wanting team to die, pretty sure he only wanted you

On side note, nikolascp @ uswest.battle.net afk'd firsts 3min of game and then was first leave. Nobody even seemed to notice. Most likely served his autobanex so no ban required.
