16mins throws me into enemies and gets me killed
21mins feeds and then tries to play it off on me talking about he was expecting me to help even tho i didnt charge or ping
31mins tries to throw me into fountain but failed and threw a creep, he walked over to me and TRIED to throw me......this shows his agenda

Didn't watch the replay, but the game ends at 31 minutes... so you have one viable ban reason, him throwing you into enemies at 16 minutes.

Assuming he actually did expect some assistance at the 21 minute mark, that is not intentional feeding.

00:16:19 He tossed you to the opponent's hero and didn't participate in the team fight, walks back and it's not a good strategy to toss in the first place.

00:21:36 Tiny saw Drow Ranger and Slarder, in the middle lane, and attempts to fight both of them, which obviously he's going to get killed was a silly decision to even fight.

00:21:48 Tiny is dead.
