Any further thoughts:

Four attempted vote kicks on axe w/o justifiable cause. Two initiated by this guy.
Sure, he had zero kills and 5-6 deaths at time of initial vk but given his stats it wasn't unlikely to happen. Plus he was the "tank" hero.
He was there practically each team fight/gank from what I can tell.

Wouldn't have bothered submitting had the cleaner not wanted to be banned

On side note. If you wanted, you could probably ban Axe for flaming but that is about all I saw after reading chat.

I think the side note is important actually. His chat and attitude towards his team mates, plus his attempted ghosting of ursa, plus his play, make a good case for game ruin. Since opposing players could not see the chat and game play outside of fights (creeping woods, not helping team mates, etc), the game ruining was not apparent to you.
