Reviewing now.

Checked the timestamps you claimed and from what I could see he was helping the team, the fight at top he came from behind and then teleported straight back to the tower when he respawned to help again.

Unless you can provide more specific timestamps of him refusing to help I'll have to deny this request.

Example :
21:45 - Runs to neutral camp rather than pushing mid with team
etc.
