Any further thoughts: he kept pushing and dying alone 1v5 and didn't join our fights...

I didn't see any clear evidence of refusing teamplay, only at 46:30 he teleported to top lane, however the fight happened at mid lane which result losing mid rax. He did try to help by pushing the lanes over, so he can rax the enemy when they were engaging in the fight. Hoever, he probably just chose a bad timing to push or duel which is not a reason to be banned.
