Time of incidents (Note in-game timer or replay timer): approx. 6 mins and 14 mins in, plus a host of other fishy stuff throughout game from other teammates
Any further thoughts: placed a bunch of sentries to make sure there were no ob wards around...found none. Teal made a direct line to me in forest twice with no prior sight (i.e. running from lane with vision) Maybe it was luck, but it looked very suspicious (or was someone else on team)
