Well, he had a ward up there, so of course he got all those runes. 15:40 to 15:50 (replay timer), he dodges AA's spell (well, tries to but it is too late), who is clearly in fog. Could be luck though.

The timestamp you give, which is ingame-timer, doesn't seem to be a suspicious move by him.

Leaving it for other mod(s) to decide.

This is correct. AA ulted me in the lane almost every time his cooldown was up. I tried to juke it when I thought he might be using it. Sometimes got lucky. Most times didn't.

Well I watched the replay, the evidence of so called mh is not concrete enough to prove said player mh. Unless there is a specific time point which I may have missed I will be denying said request.
