Please provide timestamps regarding throwing and maphacking as mentionned by @noobility , or your request will be denied.

both times stated above, 56:11 and 64:45 are times where egm(rubick) made his maphack obvious.

Can you elaborate...? What did he do? Why are his moves suspicious? Did you review the game before posting this ban request? Did you find any other time stamps? 2 instances will hardly be enough to ban someone.

16:31 Too many suspicious fog clicks which most of them are delayed.
16:32 EGM; (Rubick r fog clicked Earthshaker).
18:46 They commenting on (Rubic's Lothar's and acussing him of a mh). Because: He run at Leragas Vile (Spot) at 18:37
18:51 EGM selected Undying: Liberte
19:28 EGM selected (Earthshaker):
20:21 Creep attacks you. There were no maphack detected at that instance. Use !ignore command if someone is annoying/flaming you dont need to reply on every word.
27:2 Rubick clicked Storm Spirit. They (3) engage him at neutral spot.
35:33 Rubick r clicked Storm Spirit Hmmm... He've been at (Secret Shop).
41:2- Undying farming neuts, while his teammates pushing bottom lane. After that He kept push. He was farming through the woods for about: 20 minutes even, longer.
47:30 He tp bot and then go back to farm in woods.
49:01 Finished: Assault Of Cuiras and goes back to neuts instead with team.
55:57 There was a struggling and a fog click (56:11 nothing found there). Was ran back to their base.

About EGM someone need to review/confirm the fog clicks time stamp / instances.
For the all records, he has no vision/wards

Althought, the fog clicks and timestamps I can't disagree. Most of those fog clicks are not delayed and some are delayed.
