Go here first, click on [New Topic*] and you will see this page.
Example brq:

Replay Link: https://entgaming.net/findstats.php?id=6424460
Game Name: [ENT] Island Defense #16
Your Warcraft III Username: sat
Violator's Warcraft III Username: darkness09
Violated Rule(s): suicide as titan
Time of Violation (in-game or replay): 10:45
Any further thoughts: (You can usually just delete this line completely, unless you have something useful to say. Most of the time people use this to make some dumb remark which is unnecessary and needlessly dramatic. So reserve it for actual insight.)

To find the Replay Link and Gamename and the violator's username and realm, you go here and type your username into the User Search. That will lead you here. From there you click on the correct game link, and that link is what you paste into Replay Link, as I showed above. From there the rest is self explanatory. When someone is reviewing the game they will click Download in the top right of that link and from that page they can see chat and get a copy of the actual game, but you don't need to worry about that except when you are searching for Time of Violation, which mods like to have. If you do not put a specific timestamp, even if you put a range of time, many mods will not even check the replay and deny it. So put in that effort.
