16:05 - jumps straight at Maiden who is in the fog
20:58 - perfect jump over the land mines, although mines could not be jumped over
22:00 - activates ulti and goes back milisecond after Doom appears near him
31:25 - Slark stands near the secret shop. A flying crow approaches the secret shop (not visible to Slark), he moves towards the shop before the crow becomes visible to him, and quickly activates mask of madness and kills the crow
44:00 - jumps directly at an enemy ward and destroys it since he carries a gem, without having a vision of it before jumping. Note that his Nightcrawler ability didn't turn off at all, which could indicate an enemy observer ward
46:55 - starts to go back before invisible Doom appears, which looks like he is afraid of Maiden
47:30 - he is not afraid of Maiden any more, so goes straight to her, kills her and he does his famous "land mines jump" again at 47:50
...

That should be enough for a ban.

Any further thoughts:
That player has played more than 400 DotA games on ENT. There have already been MH reports against him before:

16:05: land perfect leap at maiden who has been in fog for quite some time.
20:58: leap over the land mines. He had no vision of them. I went back to when techies placed them and there were no sentinel heros nearby that would have given vision of their placement, so he had no way of knowing they were there.
22:00 To be fair, all sentinel heros were not visible on the minimap at that time, he was solo creeping, and ults and retreats as soon as the creep wave is killed, but also as soon as doom appears. This could be prudent retreat, but why ult? Unless he saw doom coming and was avoiding dooms ult?
31:25: turns towards incoming crow before having vision of it, then activates mom and kills it.
44:00: He could have just been dewarding and leaps to the common spot wards are placed, but he has gem, so why not just walk up to it and look first?

I think these incidents are indications of the use of maphack.
