I'm 100% sure you left it. Otherwise it would say that you lagged out and there was also no lag-screen in the game. Just you suddenly disappearing 3 secs before it was your heal. Even if your WC3 or PC crashed, it wouldn't just show "left voluntarily".
Also your ban-history shows 18 bans by autoban and your leave-rate isn't 0% either.
