Bans usually fall between the lines of three to seven days in length, and the conditions for which a ban will commit falls into the hands of a moderator, or admin. However, I believe that bans, or punishments in place thereof, would benefit from gametype-based lengths, preset to the custom game being hosted. I've seen this work effectively on a few different hosting engines, not necessarily on a WC3 hosting bot. It would be nice to implement this feature, or at least look into it.
I don't want to ramble. Please feel free to comment, I really hope we can do something about this issue. Frequent flyers in the trolling world get away with three-day bans too frequently.

The intention and the act negatively impacting the game and the ruined gaming experience of others is what matters.
Although we have set rules, cases vary by intention and impact. We do have a general range for each bans, but the actual chosen length of the ban is indeed decided by the game moderator. If the ban is too long, user may appeal for reduction or an admin will adjust the ban.
The length could be considered (how many games ruined), but not one game.
The purpose of a ban is for rule breakers to learn and play games normally after. If they repeat the same offense, bans are longer.

I suppose so, with such a thinned-out community as it is, it may be impractical...
Do moderators have to report a ban to a ranking admin before a punishment's issued, or do the ENT bots do it automatically? Just curious...

The community posts ban requests. Similarly, we have a ban appeal section for those who would like to appeal their case.
Once a topic is reviewed by a mod, he/she will process the topic and apply a ban if it is necessary.
If a moderator is in the game playing with others, he/she will be able to ban a violator.
