Describe any relevant experience, both in moderating and in Warcraft III: been with this map since years ago, when id.net still active. Well enough to tell if someone is maphacking thru viewing replay, etc.
Why do you want to become a game moderator: 1) had enough with idiots who left as titan during countdown, and repeat, 2) ability to kick/ban obvious maphacker on the spot
What will you bring to the ENT/Warcraft III community: better gaming experience for everyone

How do you know who an 'obvious maphacker' is. Regardless you won't get through with that little effort in the ban requests section. Admins have no idea who you are and what you're capable of.
