Add Bot Name to Thread name for Ban Request

I'm not totally sure of how your game moderators work. Whether they're mod for all bots or only certain bots. If a game mod is for all ENT bots then ignore this suggestion. If game mode is only for a certain bot then this suggest applies.

Suggestion is to add the bot name to the thread so the mods can easily browse through the list of threads and find the one that they are working on.

Existing: Name @ realm
Suggestion: Name @ Realm on Bot
Example: boowoo @ US-East on ENT37
