Botname field should be removed. This really doesn't have an impact on anything and 99% of the time people have no idea which bot it is.

yes, I think we should change the appeal...
Should have this link, ppl will check why they are banned, if they don't know how to use ent link*of course*
Plus if they don't know the gamename they should atleast put some info to describe what kind of the game because sometimes you have to download the replay or check his nickname in the database to find out what game he is playing. Example this request, some admins only handle non-dota game. :3

Also, possibly changing "gamename" to "mapname or gamename", it would help for the people that can't seem to figure out we don't need them to type their name twice thinking that gamename means their personal username for some reason.

Updated the form and added links to both ENT Link and the banlist. ENT Link should work without account validation if they're banned by IP address.
