I watched it. I tried to be the most precise possible. Overall, Riki never got ganked by surprise. His deaths were during team fights and failed attempts to kill someone alone (greedy).

Minor @ ~7:15 Wyvern escapes enemy through the trees and hides there a little while. 2 times riki attempts to reach him. Second time, Riki exactly faces Wyvern. He then says ''he has no tp, he will come out; then says ''Jk'' as soon as Wyvern tp's out and then moves back to lane.

Medium @ ~18:40 Krob moves back as Tinker dies, runs towers the trees and hides to TP. Riki moves towards Krob and decides to move back as soon as Krob teleports away (no vision).

Suspicious @ ~28:00 (DD appears) While Wyvern is in scourge woods farming, riki takes DD and goes directly to him. Wyvern hides in trees and riki exactly goes to his spot. Wyvern ports out.

Suspicious @ ~29:40 Riki heads top, runs pass Wyvern with gem, ignoring him then moves farther in the lane, avoiding him and Earth.

Medium @ ~29:00 Earth runs to top rune while Riki chases him. In my personal opinion, Riki seemed convinced that he ran to the rune hence he headed straight there without hesitation while I personally would think he's long gone.

Suspicious @ ~34:50 Wyvern heads to secret shop while Riki being mid then Riki activates mom and charges towards Wyvern at the shop to kill him.

Medium @ 36:30 Riki heads top tower while Wyvern is also heading there, appears directly on his spot and kills him. I personally would never go to tower without a proper reason. Even if brown was there, I would not dive the tower and risk to get ganked.

Medium @ 45:15 Riki chases Wyvern without vision.

Blatant @ 55:40 Riki attemps to kill Ursa, Ursa uses loth. Riki follows him and meanwhile he activates his MoM. Kills Ursa when he attempts to TP. This confirms it all for me.

On your 28 min time stamp, not only does he move straight to ww in woods, but he activates mom prior to gaining vision, then he doubles back mimicking ww change of direction, and then moves directly to him within the trees as he tps out.

On your 4515 time stamp sa has vision of ww from a ward.

On your 5540 time stamp not only does he follow ursa's changes in direction but activates mom to close on him without vision.

Your other time stamps are accurate as well.
