Any further thoughts:
SF gets mad that he didn't get mid, and at around 0:22 just as game began says "means free lose, have fun" and then "u will see, 100% lose".
Uses !ff 3 minutes into game after first death. Then uses !ff twice at 7:00, telling team "ff you noobs it's over, I already told you" and constantly ff ing throughout game. Uses again around 9:00 and says "WR you lost this game when you picked mid" (I was 2-1 at the time, not feeding like him).
Basically I think this guy just got upset he didn't get mid and threw the game, making it clear we were going to lose because of how the lanes were picked. Even if he didn't throw on purpose, there's enough grief and abusing !ff command for a ban I think.

None of his deaths looked intentional. You guys got rocked by a very good slardar. The chat, though annoying, is not ban worthy, and it did not reflect his game play, as he was active in ganking and defending.
